Comprehensive Care

Older adults and their families are increasingly interested in services that allow them to remain safely in their homes and communities, even after health issues begin to interfere with their daily functioning. An innovative group of programs from CenterLight Health System’s CenterLight Healthcare do just that for seniors who qualify for, but want an alternative to, nursing home care. For instance, CenterLight Healthcare offers the nation’s largest Program of All-Inclusive Care for the Elderly (PACE). PACE provides seniors with complete medical care, rehabilitation therapy, and recreational therapy in our day health centers, and free medical transportation. It also provides home care.

 


For adults who prefer to keep their own health care providers, we offer CenterLight Healthcare Select, a New York State Medicaid Managed Long Term Care Plan. This program offers skilled nursing care, social work services, medical transportation and other health-related services, such as vision and dental. CenterLight Healthcare Select does not cover physician care or hospitalization.

We also offer CenterLight Healthcare Direct, a Medicare Advantage Special Needs Plan for people enrolled in Medicare Parts A and B. This program provides primary physician care, specialty care, related medical services and a Part D drug benefit to those who meet the eligibility requirements.

We offer (PACE) to individuals living in our service areas, which include the Bronx, New York, Queens, Kings, Richmond, Westchester, Suffolk and Nassau Counties.
