Adult Day Health Care FAQs

What services do you provide?

In addition to healthcare monitoring, physical, speech and occupational therapies, the program’s healthcare professionals provide a variety of services including podiatry, diabetes management, nutritional counseling, therapeutic recreation, social work services and assistance with personal care and activities of daily living.

Who is this program designed for?

Our mission is to serve individuals who require assistance on a day-to-day basis and have a desire to continue to live at home. Our program is offered to adults 18 years and older who are under the care of their doctor for a variety of chronic medical conditions requiring nursing care, rehabilitation therapy or assistance with daily living activities.

Can I still use my personal doctor?

Yes. Your doctor will be part of your plan of care. If you do not have a personal doctor or primary care doctor, we will be happy to recommend one.

How many days must I attend the program?

Participants must attend the program at least one day per week but may attend every day if they qualify. Our staff is there to accommodate you and create a personal schedule tailored to your needs.

Is transportation provided?

Yes. We provide door-to-door transportation in wheel chair accessible ambulettes from anywhere in Queens.

Who pays for Adult Day Health Care?

The costs are covered by Medicaid, some insurance plans or through private payment.

Is there a nurse on staff?

Yes. Our registered nurse will monitor your blood pressure, help manage diabetes or other chronic conditions, monitor vital signs and administer your medications while at the program according to your doctor’s instructions.

Will I lose my home attendant if I enroll in the Adult Day Health Care program?

No.

Does your staff speak other languages?

Absolutely! One of the great advantages of our program is not only the ethnic diversity of our participants but of our staff as well. Our staff can translate and facilitate communications with doctors and other medical service providers. Our staff speaks multiple languages and dialects reflecting the diversity of our members including: Bengali, Bukharin, Cantonese, Creole, Farsi, French, Fuzhounesea, Gujarati, Hebrew, Hindi, Mandarin, Punjabi, Russian, Shanghainese, Spanish, Tagalog, Taiwanese, Urdu, Uzbek and of course, English.

Do you accommodate special dietary needs?

Yes. Our program has a registered dietician on staff and we accommodate special dietary needs for medical, cultural and religious reasons. This includes nutritious meals prepared for those requiring low sodium, low cholesterol and sugar free diets, as well as Kosher and vegetarian meals and culturally specific choices including, but not limited to Chinese, Russian and Indian cuisine.

Can someone help me with Medicaid information and application?

We will be happy to assist you in applying for Medicaid. Our program has an experienced social worker who will advocate on your behalf.

Can a family member accompany me to the program?

We encourage a family member to accompany you on your initial visit.

Do you provide assistance with scheduling medical appointments?

Yes. We often schedule medical appointments for our clients.

What kind of recreational and special activities do you offer?

We offer a wide variety of activities including exercise such as Tai Chi and Yoga, entertainment, trips, parties, crafts, mahjong and discussions on everything from current events to holistic healing.

How soon can I start?

We will be happy to enroll you as soon as your Primary Care Doctor completes our medical report.
