What is Medical Model Adult Day Health Care?

Medical model Adult Day Health Care (ADHC) is a licensed community-based day care program that provides health, rehabilitative, recreation and social services to those who need assistance to live independently. It allows individuals to continue to live at home and avoid placement in a nursing home.

While the Adult Day Health Care programs are wonderful for participants, they are also vital for caregivers, enabling them to:

  • Work outside the home
  • Avoid placing a loved one in a nursing home
  • Take a break from what is often a 24-hour responsibility
  • Share the physical work involved with caring for a loved one
  • Maintain and enhance their other relationships and interests

CenterLight Health System’s Adult Day Health Care Programs change how one lives, not where they live. At our centers, we offer a wide array of services and activities to enrich each day, enhance health and foster independence. We have seven locations throughout the Bronx, Brooklyn, Queens, and Westchester — many of our sites are open six or seven days a week and some have evening hours. We provide door-to-door transportation designed to make it easier for both participants and caregivers.

Our programs serve a richly diverse community, with a highly skilled staff representing many languages and cultures on hand to provide individualized care for each participant’s physical and mental health needs. We give you peace of mind while caring for your loved one.
